I don’t know the chipmaking process, but pretty wild for the incorrect shaped inlay and denomination, right?

but the inlays are different shapes for 100 and 500, so it was more of a printing error wasn't it ?
 
I'm guessing an entire sheet of $500 label misprints was printed/cut (and then caught), with one (or more) of the error labels mistakenly making it into the production bin.

Getting it onto a chip slug is just a matter of grab-dab-n-go with no time for visual inspection beyond centering, before heading to pressing. This step is today mostly automated (but not during the Dunes era).

Most error chips are caught after pressing and/or finishing.
 
Just out of curiosity… what is a crazy offer? This whole hobby throws money around like it’s Monopoly, so everything has been crazy imo for several years now. LOL
Yeah but out-crazying @doublebooyah85 himself would be a near insurmountable level of craziness. Even his avatar is the pinnacle of psychotic.
