Okay I picked up some more of these babies. From Mortimer's Card Room in Marina, CA. According to the ChipGuide:

"Closed because General Manager and wife stole from the card room while the owners were on vacation."

These are manufactured in 1986, so pre-USPC. They are used but in very good (not great) shape. They are pretty flat.

The setup boxes are in good shape but are marked (see pictures).

Price: $6/setup plus shipping. Note: I can fit 2 setups in a SFRB. Beyond that, I've no idea what shipping will cost.

I have 0 setups (was 22) but I can get more if the demand is there.
