Looking for the designs/person who designed these MSK plaques:

I'm very interested in trying to get a GB together for similar plaques but labeled "JACK Cincinnati". Not sure what the custom is with regards to this - whether I need to pay a fee to the designer or an artist or something. Any guidance would be appreciated.
