Most obvious reliable tell you've ever seen (3 Viewers)

I hate sunglasses at the table. Exit Only Lounge in Colorado/Dhahran enforces a no sunglass rule.
That's funny haha. I actually like wearing sun glasses because it gets on people's nerves. It's become a poker faux pas. Trolling perhaps or advertising my noobness.

Trihonda can attest to this when we were in Vegas :).

I don't wear them in home games though...
 
I have no problem with any sort of glasses, hats ect ... Everyone from Johnny Chan, Chris Ferguson to Stu Ungar, John Juanda & even Hellmuth all wore/wear them...
I do have a problem with the people sitting at a tournament with an Ipad, laptop or tablet, eaphones in ear, watching an entire movie, or constantly playing FaceSpace or MyTime or twitbook during the tourn ....
 
Most reliable tell, when I push all-in and they call. I know before they even turn their cards over I am beat, I immediately start counting out my chips or stand up and start packing my stuff because I am done.

At the WSOP, they allowed players to play poker on their tablets while in a live game. Initially I hated it, until I realized the guy was basically showing me how he played every hand. I knew his range within a few minutes (because hands play so much faster online), and knew he loved to fire big when he was on a draw (even 3 betting). If it wasn't for the other 8 players at the table, that guy could have made me quite wealthy.
 
The most reliable tells I've seen fall in the general class of the "reluctant raise," a subset of weak-means-strong. This includes:
  • Mini-shrug or slightly upturned hand in the course of a betting or raising motion (as if to say, "What the hell, may as well bet").
  • Actually saying, "What the hell, may as well bet," "I think I have to raise here," et al (raise with an excuse).
  • Basically any sound a person might make when disappointed, followed by an aggressive action.
This class of tells is usually dead-on with weak, inexperienced players. Not so much with anyone who has meaningfully studied tells.
 
The former host of our weekly game would count out chips, then check. If he did that, ANY bet would take the pot. Every time. He did it a lot when he was "giving up" a weak hand or bluff. He no longer hosts because he can't seem to win anymore.
 
In regards to hands trembling, back when I first started playing at the casino I ran into a much older gentleman who sat down at the table and immediately 5 bet me preflop. Me being young and still optimistic about my abilities, I kept betting into him because his hands were shaking wildly. We had it all in by the turn, me feeling great about my pocket QQ on a 10 high board. He had AA. He sat for three more rotations, and never played another hand.
Towards the end of the night, I remarked about his shaking to a regular. He starting laughing at me, and informed me the man had been exposed to agent orange in the Vietnam War. So much for picking up a tell.
