I wonder if we're not seeing everything here. They almost always charge some sort of "administrative fee" on top of the tolls, specificially to cover postage and other costs like this. Here it's $2.50 per billing statement.

My father in law lived with us before he passed 8 years ago. Each month we receive a bill for $0.52 in his name from an insurance company. It’s been 8 years now and still we receive the bill- we have called, but it doesn’t seem to matter. Oh well.
